The whole time Skye is at Lewis' apartment, he is acting extremely weird. Unbeknownst to Skye,the Smile Entity is playing tricks on Lewis, which leads him to smash his head with a weight right in front of her.

Smile 2’s huge twist was really well-executed, but it sets up a potential problem for Smile 3 regarding further twists and audience expectations.

While vomit may not contain an ideal sample of DNA at all times, the detectives who investigate Lewis' apartment will without a doubt be able to identify the vomit as Skye’s, which would make her a suspect in his murder.

However, in a panic, she flees the scene without erasing any trace of herself. Additionally, Skye also chooses not to alert the authorities ofLewis' death inSmile 2. This would make Skye look even more guilty when the police discover Lewis' body. Being connected to a drug dealer’s death could easily ruin Skye’s reputation and career, so she searches for whether DNA can be found in vomit.

The Police Never Contact Skye In Smile 2
Even though Skye is worried about having left her vomit behind at the crime scene, she is never contacted by the authorities inSmile 2. When Morris quite easily connects Skye to Lewis' death, it is odd that the police never reach out to her within the movie’s runtime. It also would have been very easy to find Skye, since she was performing at a heavily advertised concert just days after Lewis' death. Therefore, it is confusing that the police do not show up at the concert or during another scene.
This Smile 2 Moment Clarifies How Much Of The Ending Is In Skye’s Head
The monster in Smile 2 manages to trick Skye and the film’s audience with its powers of illusion, but one scene shows how much of the ending was real.
Perhaps the police just did not track her down quickly enough before her death inSmile 2,butSkye’s vomit seems like an unresolved plot point. There is a lot of emphasis placed on the vomit near the beginning of the film, but once Skye’s hallucinations become worse, the focus of the movie completely shifts to her being tormented by the Smile Entity. Therefore, other than being a method to transfer the Smile Curse from Lewis to Skye, the circumstances of Lewis' death were ultimately not very important inSmile 2.
